Preschool/Headstart Information

Please complete all six forms below.  Remember, once you start a form you must complete it. The form will not save your information for you to return too. You will have to start the form over if you can not complete it. You do not have to complete all the forms at once but please have them completed by your registration date. Please bring the following: Child's Birth Certificate, Proof of income, Child’s Medical card and Social Security card
